Hello, I understand that you need help with a lookup that has multiple lookup criteria, right?
Excelchat Expert 08/10/2018 - 12:43

If those don't work, your network may be partially blocking our platform and if that's the case I'd have to ask you to please contact our support team so they can help you with the connectivity. Meanwhile I'll be solving your problem based on the information and the sheet you provided.

Hopefully this session will still be rated fairly as there's really nothing I can do about the network blockage.

Please see the SOLUTION to lookup based on multiple lookup values in the SOLUTION Sheet cell G3.

The formula is
Excelchat Expert 08/10/2018 - 12:47
=index($C$1:$C$10,match(E3&F3,index($A$1:$A$10&$B$1:$B$10),0))
Excelchat Expert 08/10/2018 - 12:48
This concatenates the ID and Name and then looks it up based on those 2 criteria.

Here's the file with the solution.
