I need to know how I can post a survey with question containing data from an excel column

In the shared example, it would be , "Is your name Jane? If yes provide 4 numbers" and these 4 numbers would go to 4 adjacent columns

yes its pretty much how it should be but rather, this would involve a survey type questionaire though, where the question would have D2, Answers would be E2,F2,G2,H2

Even if you have some fixed names then you can create a dropdown of the name's list too
