I need a formula to lookup a value from a multiple occurring list only if it corresponds to a particular month of the year

I have a list of IPs in Column A that maps to Ports in Column B. In a separate worksheet I want to perform a lookup and every time I find the IP in Column A, populate all the corresponding Ports in Column C.

The data below should return all the data from columns K,M,O and Q from the tab “all” However, my formulas are only returning data from column K in the tab "all". I am looking for a formula to look for a column that corresponds to a date from one cell, if the date matches the column header, the formula then looks for all the cells which have a value of "A" and gives me the result of the "name" that has an "A" beside it.
